Due to delays associated with refunds, if you are not certain whether a record is filed with the Leominster City Clerk, call the office for confirmation at (978) 534-7536 before ordering. It is helpful if you provide us with a telephone number when ordering so we may contact you with any questions.
If the birth record is restricted (individuals whose parents were not married at the time of birth), then you need to fax us a copy of the identification of the subject of the birth record. If the subject is a minor, one of the parents listed on the birth record must request the record. Restricted records can only be mailed to the individual who is the subject of the record or one of the parents listed on the birth certificate.
Marriage licenses cannot be obtained online. Both parties must appear in person to obtain a marriage license at least 3 days prior but not more than 60 days in advance of the wedding date. If you are ordering a copy of your marriage certificate, keep in mind that marriage records are kept where you applied for your license, not in the city or town where you were married.
Death certificates can only be issued for individuals who died in Massachusetts. They are filed in the city where the death occurred and in the city where the decedent resided at the time of death.
We are only able to process online dog licenses if we have a current rabies certificate on file. If this is a new dog or we do not have a current rabies certificate on file, please fax to (978) 534-7546. If your dog’s license is not current and late fees apply, you cannot license online. Please contact us at 978-534-7536 and we will provide you with information on late fees.
