IMMUNIZATIONS AND EPIDEMIOLOGY
Leominster Board of Health is a distribution station for vaccines
and immunizations provided by the Massachusetts Departmant of
Public Health. Leominster physicians look to the Board of Health
for vaccine supplies of tetanus/diphtheria pertussis, tetanus/diptheria,
polio, measles, mumps, rubella, hepatitis B, pneumonia and flu
vaccines. The Leominster Board of Health follows the
state-supplied vaccines and patient eligibility criteria.

IMMUNIZATION CLINICS
The Leominster Board of Health offers an immunization clinic on
Thrusdays, from 3:00PM to 5:00PM, for Leominster residents 18
years and younger. A vaccine request form from the school and/or
all past immunization records must be brought to the health
department for review. A person under 18 years of age must be
accompanied by a parent/guardian. Please call 978-534-7533 ext.

COMMUNICABLE DISEASES
The goal of the Leominser Board of Health in the area of
epidemiology and immunization is to assist the state in reporting
and investigating outbreaks of a communicable disease within our
community. The availability of educational materials and services,
as needed, are provided to assist in reducing communicable
diseases with the City of Leominster. The communicable disease
process begins when a confirmed report of a disease is received.
