Absentee Ballot Application

Absentee Ballots are available in the City Clerk's Office up until 12 pm the day before an election.

If you are unable to come into the office to complete your application you may print out the application, and mail it to the City Clerk's Office, 25 West Street, Leominster, MA 01453. Once we receive your application we will mail you a ballot. Please allow yourself enough time so we can mail your ballot and receive it back from you by Election Day.

Absentee Ballot Information for Permanently Disabled Voters

A voter who will be unable to cast his/her vote in person at a polling place by reason of permanent physical disability may file once with the City Clerk a certificate executed by a registered physician who is personally acquainted with the voter and aware of his/her permanent physical disability, stating that it is reasonably certain because of the permanent physical disability that the voter will be unable to cast his/her vote in person at the polling place on the day of the election. A voter who is permanently disabled and files a physician's certificate with the City Clerk shall be maintained on a Permanently Disabled Voter (PDV) List, and the voter will not be required to file another certificate with their future Absentee Ballot applications.

The City Clerk will mail to all permanently disabled voters a completed Absentee Ballot Application at least 28 days before an election. The Voter must complete sections 1, 2, and section 3 (if applicable), and return the signed AV form to the City Clerk allowing reasonable lead time for the Clerk to mail the ballot to the Voter and for the Voter to return the ballot to the City Clerk, which must be received no later than 8 pm on the day of the election. To be added to the PDV List, the Physician's Certificate must be returned to:
